Who needs to complete the Questionnaire?

This questionnaire is only for those Novartis suppliers who have received written notice from Novartis specifically requesting completion of the survey.

How do I complete the Questionnaire?

If you have Microsoft Excel...

• Download the Excel file on your computer.
• Fill it out electronically.
• Save the file and e-mail it to the address you received in the letter from Novartis.
