Who needs to complete the Questionnaire?
This questionnaire is only for those Novartis suppliers who have received written notice from Novartis specifically requesting completion of the survey.
How do I complete the Questionnaire?
If you have Microsoft Excel...
- Download the Excel file on your computer.
- Fill it out electronically.
- Save the file and e-mail it to the address you received in the letter from Novartis.

- Download the PDF file.
- Print the questionnaire.
- Complete the questionnaire by hand.
